Read the following carefully before admitting any Patient

ADMISSION NORMS AND CRITERIA

  • The duration of the treatment is 180 days (Minimum).
  • After admission, first 30 days client’s family members will not be allowed to speak to the client’s over telephone nor can meet the clients. Family interview / meetings with the clients will take place only Sunday once in a every month. Interview will not be allowed until permission is granted by the KHARDAH PRAYAS CENTRE authorities. Discharge is also subject to approval of the competent authorities of the KHARDAH PRAYAS CENTRE.
  • If at any point of time during the treatment at KHARDAH PRAYAS CENTRE Doctors, Counsellor, Medical staff and Authority declares that the Patient is fit and healthy, in such a situation the Guardian will be bound to get the clients discharged from the KHARDAH PRAYAS CENTRE on that very occasion.
  • Only dry food, fruits & snacks can be given to the clients – no cooked foods are allowed (cooked foods can be given with period of permission from the authority for the whole community – community rule)
  • KHARDAH PRAYAS CENTRE take no responsibility for the preservation of clothes, etc., and for the custody of valuables thing and ornaments etc. used by the clients. We understand that ornaments and valuables things should under all circumstances be removed by the guardian before the clients is admitted at the KHARDAH PRAYAS CENTRE and no money should be given to the clients for personal expenses.Towel / Gamcha / Bedsheet / Toiletries / Lungi has not to be given directly to the patient.
  • Client’s family members must attend online / physical FAMILY THERAPY and SUPPORT MEETING every week on Sunday ( 5 PM to & 7 PM). Four meeting attendance per family in a month is mandatory.
  • Charges for Medical Tests / Medicines / all Doctor Fees and Toiletries as and when required will have to be paid extra at actuals against bills. The amount has to be deposited in advance in our accounts department or through bank transfer.
  • If Doctor’s Certificate is required by the clients family, extra charge fot the same will be Rs. .......................... per certificate. Psychometric Test (not mandatory) if required as per psychiatrist, charge for the same will be Rs. .....................and the amount has to be deposited in advance in our accounts department or through bank transfer.
  • During client’s meeting family member cannot feed the clients with homemade food items/ Only Toiletries & Clothing can be given. No Mobile Phones, Laptops, Ornaments, Valuable things and Money can not be given to the patient.
  • Any legal documents like Divorce papers, Property related documants and Banking Cheque Book which clients refuses to sign cannot be signed inside the treatment centre.
  • If the client’s absconds from the KHARDAH PRAYAS CENTRE or leave the centre without consent of the authorities then centre authorities will not be held responsible for the client whereabouts/welfare outside the centre as well as client’s belongings left behind the KHARDAH PRAYAS CENTRE and will be automatically terminated from his / her treatment and there will not be any refund of fees.
  • If the clients needs to go out for parol (one day home visit), any office work, any education related work in college or university or anywhere else, consent letter has to be given to our office by the family and the clients. The TA / DA has to be borne by the family which has to be deposited in advance in our accounts department or through bank transfer.
  • I hereby has been informed that the doctors will visit accurring in this centre once in a week / 3-4 times in a month and for emergency purpose. In case of emergency situation infrastructures of this centre are not sufficient and this KHARDAH PRAYAS CENTRE not hospital/ nursing home and does not have the facility / equipment to deal with any kind of serious ailment. If during the treatment at KHARDAH PRAYAS CENTRE the health condition of the clients deteriorates then may have to be shifted to nearby hospital / nursing home or any other institution according to our choice. I will not blame the KHARDAH PRAYAS CENTRE for such transfer and will take full responsibility after admission at such hospital / nursing home or any other institution. I will try to reach as early as possible in case of emergency situation, whenever informed for discharging the clients from the centre after clearing all outstanding dues, if any.
  • If the clients is suffering from any chronic disease for which medicines/ medical tests / specialist doctor is required, such medicines/ medical tests / specialist doctor is to provided to the clients by their respective family members or money for the said medicines has to be deposited in our accounts department in advance. In case of hospitalization the family members has to bear all the hospital expenses and has to sign all the necesary documents related to the treatment of their clients and also we give written declaration/consent to KHARDAH PRAYAS CENTRE authorities.
  • KHARDAH PRAYAS CENTRE will not be held any responsible if the clients tries to commits / commited sucide during his treatment period at KHARDAH PRAYAS CENTRE .
  • We agree to pay the sum of Rupees...........................................................per months as a donation towards his / her treatment fees not inclusive of any special / regular medicine or prescrived medicine by the Doctor / Doctor Fees / Pathological Tests / Hospitalization / Toiletories as and if any required will be reimbursed by the family as per actual Bills. Outstation clients have to keep a security deposit of Rs. 10,000/- which will refunded on discharge after necesary adjustments. All clients are required to pay the monthly treatment fees in advance mode. Monthly fees are to be deposited along with Registration fees at the time of admission and will not be refunded if treatment is terminated.
